基于阴阳失衡理论的温灸疗法对糖尿病性
干眼症患者阴虚燥热状态的干预效果研究

on the Theory of Yin-Yang Imbalance on the Yin Deficiency and Heat State of Patients with Diabetic Dry Eye Syndrome
摘要: 目的:探讨基于阴阳失衡理论的温灸疗法对糖尿病性干眼症患者“阴虚燥热”状态的纠正作用,评估其改善患者临床症状及泪液功能的有效性。方法:将60例辨证为阴虚燥热证的糖尿病性干眼症患者随机分为试验组(n = 30)和对照组(n = 30)。对照组给予血糖控制加人工泪液滴眼的治疗,试验组在对照组基础上加用特定穴位的温灸治疗,每日1次,每周5次,连续干预4周。比较两组治疗前后中医阴虚燥热症状评分、泪液分泌试验、泪膜破裂时间及眼表疾病指数评分。结果:治疗4周后,两组患者“阴虚燥热”症状评分、眼表疾病指数评分均较治疗前降低,泪液分泌试验长度及泪膜破裂时间均较治疗前增加(
P < 0.01);且指标改善程度显著优于对照组(
P < 0.01)。结论:在标准治疗基础上增加基于阴阳失衡理论的温灸疗法,可能有助于改善糖尿病性干眼症患者的阴虚燥热状态,增加泪液分泌、改善泪膜稳定性,缓解眼部干涩症状。
Abstract: Objective: To explore the corrective effect of warm moxibustion therapy based on the theory of Yin-Yang imbalance on the “Yin deficiency and heat” state of patients with diabetic dry eye syndrome and evaluate its effectiveness in improving clinical symptoms and tear function. Methods: Sixty patients with diabetic dry eye syndrome diagnosed as having Yin deficiency and heat syndrome were randomly divided into an experimental group (n = 30) and a control group (n = 30). The control group was treated with blood sugar control and artificial tear eye drops, while the experimental group received warm moxibustion therapy at specific acupoints in addition to the treatment of the control group, once a day, five times a week, for a continuous intervention of four weeks. The scores of Yin deficiency and heat symptoms in traditional Chinese medicine, Schirmer’s test, tear film break-up time, and ocular surface disease index scores were compared before and after treatment in both groups. Results: After four weeks of treatment, the scores of Yin deficiency and heat symptoms and ocular surface disease index in both groups were significantly lower than those before treatment, and the Schirmer’s test length and tear film break-up time were significantly increased (P < 0.01); and the improvement degree of the indicators in the experimental group was significantly better than that in the control group (P < 0.01). Conclusion: On the basis of standard treatment, adding moxibustion therapy based on the theory of yin-yang imbalance may help improve the yin deficiency and heat dryness condition in patients with diabetic dry eye, increase tear secretion, enhance tear film stability, and alleviate ocular dryness symptoms.
