Research Status of Early Risk Stratification and Timing of Endoscopic Intervention for Pediatric Upper Gastrointestinal Bleeding: Clinical Perspectives in Northern Shaanxi, China
摘要: 儿童上消化道出血(UGIB)是儿科常见的急危重症。早期准确的风险分层——特别是预测患者是否需要内镜治疗、紧急输血、入住PICU或发生住院期间再出血等复合临床结局,对抓住胃镜干预时机、合理分配医疗资源非常关键。目前常用的格拉斯哥–布拉奇福德评分(GBS)主要基于成人数据制定,由于儿童的生理发育特点和特有的血流动力学代偿机制,直接套用该评分往往会高估儿童的病情。此外,陕北地区的儿童UGIB在病因上有其特殊性,主要表现为幽门螺杆菌(H. pylori)家庭聚集性感染率高、饮食结构独特,加之基层医疗服务半径大,对急诊分诊的准确性要求更高。本综述系统分析了儿童UGIB病因谱的演变特点以及成人评估模型的局限性,探讨了儿童校正休克指数(SIPA)、血尿素氮/肌酐(BUN/Cr)比值等儿科专标指标的临床实用价值,并结合循证医学证据总结了胃镜干预的最佳时间窗,旨在为建立适合我国西北地域特征的儿童UGIB风险预测体系提供参考。
Abstract: Pediatric upper gastrointestinal bleeding (UGIB) is a common and potentially life-threatening emergency. Accurate early risk stratification—specifically predicting “composite clinical outcomes” such as the need for endoscopic intervention, blood transfusion, pediatric intensive care unit (PICU) admission, or rebleeding—is vital for optimizing the timing of emergency endoscopy and healthcare resource allocation. Current tools like the Glasgow-Blatchford Score (GBS), predominantly developed from adult data, often lead to risk overestimation in children due to their unique hemodynamic compensatory mechanisms and physiological variations. Furthermore, in regions like Northern Shaanxi, China, pediatric UGIB presents distinct etiological profiles characterized by high Helicobacter pylori (H. pylori) infection rates and specific dietary structures. The extensive primary care radius in these regions imposes rigorous demands on the specificity of triage. This review systematically analyzes the evolving etiology of pediatric UGIB and the limitations of adult-based risk models. We discuss the clinical utility of pediatric-specific indicators, including the blood urea nitrogen to creatinine (BUN/Cr) ratio and the shock index pediatric adjusted (SIPA), and define the optimal “window” for endoscopic intervention based on the latest evidence. We aim to provide a theoretical framework for developing a localized, pediatric-specific UGIB risk prediction system tailored to the regional characteristics of Northwest China.
