CD4+ T与CD8+ T淋巴细胞水平作为非小细胞肺癌免疫治疗预后因素的价值评估
Evaluation of CD4⁺ T and CD8⁺ T Lymphocyte Levels as Prognostic Factors for Immunotherapy in Non-Small Cell Lung Cancer
摘要: 免疫检查点抑制剂已成为非小细胞肺癌(NSCLC)的重要治疗手段,但现有生物标志物(如PD-L1表达、肿瘤突变负荷)存在异质性大、难以动态监测等局限,难以精准预测免疫治疗疗效。CD4+ T与CD8+ T淋巴细胞作为抗肿瘤免疫的核心细胞,其水平、比值及动态变化可能反映机体免疫状态,具有潜在的预后预测价值。本文梳理了外周血及肿瘤组织中CD4+ T与CD8+ T淋巴细胞及其亚群在NSCLC免疫治疗人群中的预后研究进展,探讨评估其作为NSCLC免疫治疗预后因素的临床价值。
Abstract: Immune checkpoint inhibitors have become a crucial therapeutic modality for non-small cell lung cancer (NSCLC). However, existing biomarkers—such as PD-L1 expression and tumor mutational burden—have limitations including high heterogeneity and difficulty in dynamic monitoring, which hamper their ability to accurately predict immunotherapeutic efficacy. CD4⁺ and CD8⁺ T lymphocytes, as core effector cells in antitumor immunity, may reflect the host immune status through their levels, ratios, and dynamic changes, thus holding potential prognostic and predictive value. This article reviews the current research progress on peripheral blood and tumor-infiltrating CD4⁺ and CD8⁺ T lymphocytes and their subpopulations in NSCLC patients receiving immunotherapy, and discusses their clinical utility as prognostic factors for immunotherapy in NSCLC.
