摘要: 糖尿病肾病(Diabetic Kidney Disease, DKD)是糖尿病患者最常见的严重微血管并发症之一,约30%~40%的糖尿病患者会发展为DKD,并显著增加终末期肾病及心血管事件风险。心力衰竭(Heart Failure, HF)是DKD人群中最常见且最具危害性的心血管并发症,二者呈现高共病率、双向恶化趋势,构成“心–肾–代谢”恶性循环。本文系统综述了DKD合并HF的病理生理机制、综合治疗进展及管理策略,旨在为DKD合并HF的临床诊疗与未来研究提供参考。
Abstract: Diabetic kidney disease (Diabetic Kidney Disease, DKD) is one of the most common and serious microvascular complications in diabetic patients, affecting approximately 30% to 40% of diabetic individuals and significantly increasing the risks of end-stage renal disease and cardiovascular events. Heart failure (HF) is the most common and harmful cardiovascular complication among patients with DKD, and the two conditions exhibit a high comorbidity rate and a bidirectional deterioration trend, forming a “heart-kidney-metabolism” vicious cycle. This article systematically reviews the pathophysiological mechanisms, comprehensive treatment progress, and management strategies of DKD combined with HF, aiming to provide references for clinical diagnosis and treatment as well as future research of DKD combined with HF.
