Abstract: Pain-related attentional bias may contribute to the maintenance and development of clinical pain. However, traditional reaction time measures are limited in dissociating the latent cognitive components underlying such bias. This study combined a dot-probe task with the drift diffusion model (DDM) to examine how patients with clinical pain process pain-related words. Reaction time results showed that both patients with clinical pain and healthy controls exhibited difficulty disengaging attention from pain-related information. Accuracy results further showed that the pain group exhibited reduced response accuracy during attentional disengagement and target reorientation. DDM results revealed that patients with clinical pain showed increased drift rate, shortened non-decision time, and increased decision threshold, suggesting enhanced pain-cue processing and attentional orienting, accompanied by a more cautious decision strategy. This study provides computational behavioral evidence for understanding how clinical pain affects attentional processing.
