头孢哌酮舒巴坦钠致老年患者凝血功能障碍1例临床分析
Clinical Analysis of a Case of Coagulation Dysfunction in an Elderly Patient Caused by Cefoperazone Sodium and Sulbactam Sulbactam
摘要: 头孢哌酮钠舒巴坦钠是临床上常用的第三代头孢菌素合并增效剂。因其覆盖抗菌谱广、对肾功能损害小等特点,广泛应用于临床,因此不良反应的报道也逐渐增多。该药少见的不良反应为凝血系统异常,如出现皮肤淤点、淤斑,呕血、黑便等消化道出血表现,血尿等血液系统异常表现,实验室常常合并凝血功能异常。老年患者,常常合并肾功能不全,因此药物的不良反应发生率高,如若发生凝血功能障碍,可危及生命,应引起临床医生的高度重视。我科收治1例典型病例,结合临床资料进行了深入分析。现报告如下。
Abstract: Cefoperazone sodium and sulbactam sodium is a commonly used third-generation cephalosporin combined with a synergist in clinical practice. It is widely used in clinic due to its broad antibacterial spectrum and mild renal impairment. Consequently, reports on its adverse reactions have been gradually increasing. Coagulation system abnormalities are rare adverse reactions of this drug, including skin petechiae and ecchymosis, gastrointestinal bleeding such as hematemesis and melena, hematuria and other hematological abnormalities, which are often accompanied by abnormal coagulation function in laboratory tests. Elderly patients are often complicated with renal insufficiency, leading to a high incidence of adverse drug reactions. Once coagulation dysfunction occurs, it may be life-threatening, which should attract great attention of clinicians. Our department admitted a typical case, and conducted an in-depth analysis combined with clinical data. The case is reported as follows.
