Treating Chronic Eczema Based on Wind Pathogen Theory

摘要: 慢性湿疹以反复发作、剧烈瘙痒、皮损肥厚及苔藓样变为特征，属中医“湿疮”“浸淫疮”范畴。其病机复杂，常以湿、热、瘀、虚交结为患，然“风”邪贯穿始终，乃瘙痒与病情迁延之关键。本文立足《黄帝内经》《金匮要略》等经典，从外风袭表、血虚生风、湿郁化风、瘀血生风四个层面剖析风邪在慢性湿疹中的致病机理，并结合仲景方证，归纳疏风散邪、养血熄风、清热祛风、搜风通络等治法，为临床从风论治慢性湿疹提供理论依据。

Abstract: Chronic eczema is characterized by recurrent episodes, intense pruritus, thickened skin lesions, and lichenification. It falls under the categories of “dampness sore” and “eroding sore” in traditional Chinese medicine (TCM). Its pathogenesis is complex, often involving intertwined pathogenic factors of dampness, heat, stasis, and deficiency. However, the wind pathogen persists throughout the disease course and is the key factor underlying pruritus and disease chronicity. Based on classical texts including the Huang Di Nei Jing and Jin Gui Yao Lue, this article analyzes the pathogenic mechanisms of wind pathogen in chronic eczema from four perspectives: external wind attacking the exterior, blood deficiency generating wind, dampness stagnation transforming into wind, and blood stasis generating wind. Furthermore, integrating Zhongjing’s formula-syndrome principles, it summarizes therapeutic methods such as dispersing wind and dissipating pathogens, nourishing blood and extinguishing wind, clearing heat and dispelling wind, as well as searching wind and unblocking meridians. This provides a theoretical basis for clinically treating chronic eczema based on wind pathogen theory.
