从JAK-STAT信号通路视角审视梅毒血清固定的免疫机制

摘要: 梅毒是世界上最常见的性传播疾病之一,青霉素的问世曾一度让人类看到了消灭梅毒的曙光。但是也伴随着一个长期未解、临床意义重大且令学界困惑的现象:抗梅毒治疗后非梅毒螺旋体血清学试验持续低滴度阳性而不能血清学转阴,即梅毒血清固定,其发病机制迄今仍未厘清。越来越多的证据表明,机体免疫应答的失调是血清固定发生的核心环节。JAK-STAT信号通路作为多种细胞因子共享的信号转导中枢,主导着T细胞分化命运的决定和免疫应答的精准调控,但其在血清固定中的角色长期缺乏系统审视。因此,本文将JAK-STAT通路置于梅毒血清固定发病机制的中心位置,首先论述其对Th1/Th2分化的调控,继而分析IFN-γ基因多态性介导的信号传导差异,由此论证JAK-STAT信号转导异常构成血清固定免疫病理级联反应的上游分子基础。在此基础上,文章又构建了以JAK-STAT通路为核心的血清固定整合免疫病理模型,最终据此提出靶向JAK-STAT信号节点的免疫调节治疗策略,真正为打破血清固定的临床僵局提供了新视角、新思路。
Abstract: Syphilis stands as one of the most prevalent sexually transmitted diseases worldwide. The introduction of penicillin once offered a glimmer of hope for the eradication of syphilis. Nevertheless, a perplexing clinical phenomenon—persistent low-titer positivity in non-Treponema pallidum serological tests following anti-syphilis treatment, without achieving serological negativity, known as syphilis serofixation—continues to pose a challenge, with its pathogenesis remaining elusive. Accumulating evidence suggests that immune response dysregulation constitutes the core mechanism underlying serofixation. The JAK-STAT signaling pathway, serving as a shared signal transduction hub for various cytokines, governs the fate determination of T cell differentiation and the precise regulation of immune responses; however, its role in serofixation has long been overlooked in systematic reviews. This paper positions the JAK-STAT pathway at the forefront of the pathogenesis of syphilis serofixation. By examining the pathway’s regulation of Th1/Th2 differentiation and the signaling disparities mediated by IFN-γ gene polymorphisms, it systematically demonstrates that abnormalities in JAK-STAT signal transduction form the upstream molecular basis for the immunopathological cascade of serofixation. Concurrently, an integrated immunopathological model of serofixation centered on the JAK-STAT pathway is constructed, and an immunomodulatory treatment strategy targeting JAK-STAT signaling nodes is proposed, offering a novel theoretical perspective for overcoming the clinical stalemate of serofixation.
