Abstract: Among thyroid diseases, thyroid cancer is the most common malignancy of the endocrine system, and its incidence has been increasing rapidly worldwide in recent years. For patients with radioiodine‑refractory thyroid cancer and those with medullary thyroid carcinoma, conventional imaging modalities and therapeutic strategies are often insufficient to meet the demands of precision diagnosis and treatment. Fibroblast activation protein inhibitors (FAPI) that target FAP exhibit favorable biological properties and hold promise for playing a significant role in thyroid‑related diseases. This review focuses on the mechanism of action of novel radionuclide‑labeled FAPI and summarizes the research progress regarding their application in the diagnosis and management of both benign and malignant thyroid disorders, with the aim of providing a reference for the future development and clinical translation of novel molecular probes in the field of thyroid disease.
