基于“君相二火”与“五脏–天癸轴”整合的性早熟“三位一体”辨治框架构想

Construction of a Tripartite Differentiation and Treatment Framework for Precocious Puberty Integrating Sovereign-Ministerial Fire and the Five Viscera - Tian-Gui Axis

摘要: 近年来，小儿性早熟的发病率持续上升，中医药界把“肾阴虚、相火旺”当作核心病机，围绕着“天癸”与“相火”学说也取得了多项理论上的进展。汪受传引入“君相二火”学说，建立了分阶段病机理论，从早期“滋肾阴、泻相火”到晚期“泻心火、滋肾阴”，呈现出了治则的动态转换；鲁院军等则搭起了“五脏–天癸轴”辨证框架，把心脏“通明”的特性跟天癸的调控相联系，给“清心”治则提供了脏腑层面的整体支撑；邵征洋则是从“虚、火、痰、瘀”互结的角度出发，借助焦栀子配牡丹皮等药对，为心火亢旺的临床处理方法提供了验证。本文在把上述这些理论的内在逻辑做了系统梳理之后，提出“纵向分期 + 横向多脏腑 + 多维病理要素”三位一体的整合辨治构想，这个框架用纵向分期作纲来确定阶段性的治则，用横向多脏腑为目去调节五脏的兼夹，再用多维病理要素为径来指导精准用药，三层之间层层递进，目的就是让性早熟的辨治能从静态的分类走向动态的整合，给临床诊疗和基础研究提供一个新的理论参考。

Abstract: In recent years, the incidence of precocious puberty in children has continued to rise. The Chinese medicine community regards “kidney yin deficiency and ministerial fire” as the core pathogenesis, and has made a number of theoretical advances around the “Tiangui” and “ministerial fire” theories. Wang Shouchuan introduced the theory of “Junxiang Erhuo” and established the theory of staged pathogenesis. From the early stage of “nourishing kidney yin and purging ministerial fire” to the late stage of “purging heart fire and nourishing kidney yin”, it showed a dynamic transformation of therapeutic principles. Lu Yuanjun et al. constructed the syndrome differentiation framework of “Five Viscera - Tian-Gui Axis”, associated the characteristics of heart “tongming” with the regulation of Tiangui, and provided the overall support of viscera and bowels for the treatment principle of “clearing heart”. From the perspective of “deficiency, fire, phlegm and blood stasis”, Shao Zhengyang verified the clinical treatment method of hyperactivity of heart fire with the help of Jiaozhizi and Mudanpi. On the basis of systematically sorting out the internal logic of the above theories, this paper puts forward the concept of integrated differentiation and treatment of “vertical staging + horizontal multi-viscera and bowels + multi-dimensional pathological elements.” The framework takes vertical staging as the key link to determine the stage treatment principle, takes horizontal multi-viscera and bowels organs as the purpose to regulate the five zang-organs, and takes multi-dimensional pathological elements as the path to guide precise medication. The three are progressive, and strive to make the differentiation and treatment of precocious puberty move from static classification to dynamic integration, so as to provide new theoretical reference for clinical diagnosis and treatment and basic research.
