基于“肝肾同源”理论论治老年特应性皮炎的经验总结

Summary of Experience in Treating Senile Atopic Dermatitis Based on the “Liver and Kidney Homology” Theory

摘要: 老年(>60岁)特应性皮炎(atopic dermatitis, AD)在发病机制、临床表现及治疗方面与其他年龄段均有所不同，免疫衰老是其重要发病机制，免疫衰老与肝肾两脏密切相关，老年期AD的易发“阳位”、生殖器部位发病高、伴有非过敏性共病等疾病特点也与肝肾联系密切。“肝肾同源”理论包括精血同源、藏泄互用、阴阳互资互用等方面。余师刘拥军教授认为老年AD的病机以肝肾阴虚为本，风热之邪为标，故治疗时以“滋水涵木，祛风清热”为原则，实现肝肾同调的目的，另附验案一则。

Abstract: Senile (>60 years old) atopic dermatitis (AD) is different from other age groups in terms of pathogenesis, clinical manifestations and treatment, immunosenescence is its important pathogenesis, immune senescence is closely related to the liver and kidneys, and the characteristics of AD in old age such as the prone “yang position”, high incidence of genital parts, and non-allergic comorbidities are also closely related to the liver and kidney. The theory of “liver and kidney homology” includes the homology of essence and blood, the mutual use of storage and drainage, and the mutual use of yin and yang. Professor Liu Yongjun believes that the pathogenesis of AD in the elderly is based on liver and kidney yin deficiency, and the evil of wind and heat is the standard, so the treatment is based on the principle of “nourishing water and containing wood, dispelling wind and clearing heat” to achieve the purpose of liver and kidney synchronization, and a separate case is attached.
