基于“肺与大肠相表里”理论探析针灸治疗痤疮的临床思路

Exploring the Clinical Approach to Acupuncture Treatment of Acne Based on the Theory of “Lung and Large Intestine Being Exterior-Interiorly Related”

摘要: 本文基于中医“肺与大肠相表里”理论，探讨其在痤疮发病与治疗中的指导意义。中医认为，肺主皮毛，大肠主传导，二者在生理上相互为用，病理上相互影响：肺热可下移大肠致肠燥便秘，大肠实热亦可上熏于肺，二者循环往复，形成肺肠同病，热郁肌肤而发为痤疮。临床应用毫针、刺络放血及火针等疗法，清肺泄热以消疮、通腑泄浊以透疮、肺肠同治以化疮，实现脏腑同调、标本兼顾。该理论为痤疮的中医辨治提供了重要思路。

Abstract: This article explores the guiding significance of the TCM theory “lung and large intestine being exterior-interiorly related” in the pathogenesis and treatment of acne. TCM posits that the lung governs the skin and hair, while the large intestine governs transportation. Physiologically, they interact with each other, and pathologically, they influence each other: lung heat can descend to the large intestine, leading to intestinal dryness and constipation, and excess heat in the large intestine can ascend to fumigate the lung. This cycle forms a mutual affliction of the lung and intestine, with heat stagnation in the skin resulting in acne. In clinical practice, therapies such as filiform needle acupuncture, bloodletting, and fire needle are applied to clear lung heat and eliminate acne, dredge the fu organs and discharge turbidity to expel acne, and treat both the lung and intestine to resolve acne, thereby achieving simultaneous regulation of the zang-fu organs and addressing both the root and the branch. This theory provides an important perspective for the TCM diagnosis and treatment of acne.
