摘要: 双侧乳腺癌是乳腺癌中较少见但具有特殊生物学意义的类型,包括同步性双侧乳腺癌和异时性双侧乳腺癌。其发生并非单侧乳腺癌在双侧乳腺中的简单重复,而可能与独立第二原发、遗传易感性、共同内分泌和环境暴露、分子异质性及免疫微环境等因素相关。现有分子研究提示,多数双侧病灶更倾向于独立原发,少数病例可能具有共同克隆来源。本文围绕双侧乳腺癌的流行病学特征、肿瘤起源、发生机制、临床转化意义及新技术应用前景进行综述,以期为风险评估和个体化管理提供参考。
Abstract: Bilateral breast cancer is a relatively rare type of breast cancer with distinct biological significance, including synchronous bilateral breast cancer and metachronous bilateral breast cancer. Its occurrence is not a simple repetition of unilateral breast cancer in both breasts, but may be related to independent second primary tumors, genetic susceptibility, shared endocrine and environmental exposures, molecular heterogeneity, and the immune microenvironment. Current molecular studies suggest that most bilateral lesions are more likely to be independent primary tumors, whereas a small proportion of cases may have a common clonal origin. This review summarizes the epidemiological characteristics, tumor origin, mechanisms, and clinical translational significance of bilateral breast cancer, aiming to provide a reference for risk assessment and individualized management.
