中药熏蒸联合足浴治疗肝肾阴虚型干眼症的疗效分析

Efficacy Analysis of Traditional‑Chinese‑Medicine Fumigation Combined with Foot Bath for Dry Eye Disease of Liver‑Kidney Yin‑Deficiency Pattern

摘要: 目的：观察自拟方中药熏蒸联合足浴治疗肝肾阴虚型干眼症的临床疗效。方法：选取2024年1月至2024年12月在我院就诊的肝肾阴虚型干眼症患者182例(182眼)，随机分为观察组和对照组，其中观察组87例(87眼)，对照组95例(95眼)。对照组予玻璃酸钠滴眼液联合中药熏蒸治疗，观察组在对照组基础上加用中药足浴治疗。两组均以5天为1个疗程，治疗4个疗程后比较疗效。观察指标包括泪液分泌量(Schirmer I试验)、泪膜破裂时间(BUT)、眼表疾病指数(OSDI)评分及中医症状积分。结果：治疗后，观察组总有效率为83.91%，显著高于对照组的63.16% (P < 0.05)。观察组SIT、BUT改善程度均优于对照组(P < 0.05)，OSDI评分及中医症状积分下降幅度大于对照组(P < 0.05)。结论：中药熏蒸联合足浴治疗肝肾阴虚型干眼症，可有效促进泪液分泌，增强泪膜稳定性，改善临床症状，且安全性良好，体现了“上病下治、标本兼治”的中医特色。

Abstract: Objective: To observe the clinical efficacy of self-formulated herbal prescription-based fumigation combined with foot bath in treating dry eye disease (DED) with liver-kidney yin-deficiency pattern. Methods: A total of 182 patients (182 eyes) diagnosed with liver-kidney yin-deficiency type dry eye disease who were treated in our hospital from January 2024 to December 2024 were enrolled in this study. The patients were randomly divided into the observation group and the control group, including 87 cases (87 eyes) in the observation group and 95 cases (95 eyes) in the control group. The control group received sodium hyaluronate eye drops together with herbal fumigation, while the observation group was additionally given traditional Chinese‑medicine foot bath on the basis of the regimen adopted by the control group. One treatment course lasted for 5 days, and therapeutic outcomes were compared after four consecutive courses. Observation indicators comprised Schirmer’s I test (SIT) value, tear-film break-up time (BUT), Ocular Surface Disease Index (OSDI) score and Traditional Chinese Medicine (TCM) symptom score. Results: After treatment, the overall effective rate of the observation group reached 83.91%, which was markedly higher than 63.16% of the control group (P < 0.05). The improvements of SIT and BUT in the observation group were superior to those in the control group (P < 0.05). Meanwhile, the observation group obtained larger declines in OSDI scores and TCM symptom scores (P < 0.05). Conclusion: Herbal fumigation combined with foot-bath therapy can effectively stimulate tear secretion, improve tea-film stability and relieve clinical manifestations with satisfactory safety for patients with liver‑kidney yin‑deficiency‑pattern dry eye disease. This treatment embodies the distinctive TCM theory of treating upper-body disorders via lower-body interventions to address both symptoms and root causes.
