Case Studies of Zhang Hong’s Acupuncture Treatment for Stress Urinary Incontinence

摘要: 分析总结张虹教授运用针灸治疗压力性尿失禁(SUI)的临床经验。张虹教授将压力性尿失禁归为中医的“遗溺”“膀胱咳”等范畴，认为本病追其根本因“虚”所致——肺虚则通调失常、脾虚则气陷不固、肾虚则封藏失司、经脉失调则脏腑失养，治疗本病重在调补，调脏腑、调经络，温补气血。张教授推崇针灸治病，巧用改良的通督调神盘龙针法通督升阳、调神聚气，配合芒针深刺透穴及电针刺激经络气血，加用灸法温阳补虚，最终达到固摄止遗的目的。

Abstract: This article analyzes and summarizes Professor Zhang Hong’s clinical experience in using acupuncture to treat stress urinary incontinence (SUI). Professor Zhang categorizes stress urinary incontinence under the traditional Chinese medicine (TCM) concepts of “Yi Ni” and “Bladder Cough”, positing that the root cause of this condition lies in “deficiency”: lung deficiency leads to impaired regulation of the meridians; spleen deficiency results in qi collapse and instability; kidney deficiency causes dysfunction of the storage and containment mechanisms; and imbalance of the meridians leads to inadequate nourishment of the zang-fu organs. The treatment approach for this condition emphasizes regulation and tonification, specifically, regulating the zang-fu organs, harmonizing the meridians, and warming and replenishing qi and blood. Professor Zhang advocates the use of acupuncture for therapeutic purposes, employing a modified Tongdu Tiaoshen Panlong needle technique to unblock the Tongdu channel, elevate yang energy, and regulate the spirit while concentrating qi; this is combined with deep insertion of penetrating points using mang-needles and electroacupuncture stimulation of the meridians and qi-blood flow; additionally, moxibustion is applied to warm yang and tonify deficiency, ultimately achieving the goal of stabilizing the urinary tract and halting incontinence.
