Abstract: Tuberculosis (TB) is a chronic respiratory infectious disease caused by Mycobacterium tuberculosis (Mtb), which poses a severe threat to socioeconomic development and public health and remains a major global challenge in infectious disease control. At present, mycobacterial culture and imaging examinations serve as the primary tools for TB diagnosis and therapeutic response monitoring, yet both methods possess prominent limitations. Developing diagnostic and therapeutic monitoring techniques based on non-sputum specimens may provide an effective alternative strategy. In recent years, Krüppel-like transcription factor 2 (KLF2), guanylate-binding protein 5 (GBP5), and dual specificity phosphatase 3 (DUSP3) have been identified as host transcriptional biomarkers closely associated with tuberculosis, exhibiting great potential for TB diagnosis and therapeutic efficacy evaluation. This review systematically summarizes the research progress regarding KLF2, GBP5 and DUSP3, including their molecular mechanisms underlying TB pathogenesis, diagnostic performance, and capacity for therapeutic response monitoring, so as to offer evidence and insights for precise prevention and control of tuberculosis as well as the development of novel diagnostic and therapeutic regimens.
