Abstract: Chlamydia pneumoniae is an important cause of atypical community-acquired pneumonia in children. Its obligate intracellular lifestyle, biphasic developmental cycle, and reversible persistent phenotype contribute to an insidious onset, prolonged clinical course, time-dependent pathogen detection, and incomplete microbiological eradication. Recent studies from China and other East Asian regions have demonstrated a marked resurgence of pediatric C. pneumoniae pneumonia, predominantly among school-aged children, while infants and immunocompromised children remain vulnerable to severe disease. Clinical and radiological findings are non-specific, and coinfection is frequent. Nucleic acid amplification tests should serve as the cornerstone of etiological diagnosis and must be interpreted together with specimen type, timing, paired serology, imaging findings, and clinical evolution. Macrolides remain the first-line antimicrobial agents, but supportive care, assessment for coinfection and complications, and antimicrobial stewardship are equally important. This review summarizes advances in pathogen biology, epidemiology, host immunity and microbiome, clinical phenotypes, diagnostic technologies, treatment, long-term consequences, prevention, and future research priorities of pediatric C. pneumoniae pneumonia.
