Abstract: Objective: To investigate the efficacy of agomelatine combined with light therapy in patients with depressive disorder accompanied with insomnia, and to explore the effect on serum inflammatory cytokines. Methods: A total of 132 patients with depressive disorder and insomnia admitted to Qingdao Mental Health Center from July 2025 to May 2026 were enrolled as study subjects. All patients were assigned to the control and experimental groups, with 66 patients in both group, using a random‑number table. The control group received agomelatine mnotherapy, while the experimental group was given additional light therapy on the basis of agomelatine treatment. Compare the Hamilton Depression Rating Scale-24 item (HAMD-24), Hamilton Anxiety Rating Scale (HAMA), Pittsburgh Sleep Quality Index (PSQI), Treatment Emergent Symptom Scale (TESS) scores, and detect the level of serum inflammatory cytokines (interleukin-6, interleukin-8, tumor necrosis factor-α) at baseline, week 4, and week 8 of the treatment. Results: After 8 weeks of treatment, the HAMD-24, HAMA and PSQI scores of both groups were significantly decreased compared with baseline (P < 0.05), and the scores of the above scales in the experimental group were significantly lower than those in the control group (P < 0.05). After 8 weeks of treatment, the serum IL-6 level in the experimental group was significantly lower than that in the control group (P < 0.05), and it was also significantly reduced compared with baseline and the level at week 4 of treatment (P < 0.05). There were no statistically significant differences in IL-8 and TNF-α levels in both groups, as well as serum IL-6 level in the control group before and after treatment (P > 0.05). After 8 weeks of treatment, there was no statistically significant difference in TESS scores between the two groups (P > 0.05). Conclusion: Agomelatine combined with bright light therapy can effectively relieve anxiety and depression, improve sleep quality, decrease the serum level of IL-6 and are well tolerated in patients with depressive disorder accompanied with insomnia.
