胃复春治疗慢性萎缩性胃炎的研究进展
Research Progress of Weifuchun in the Treatment of Chronic Atrophic Gastritis
摘要: 慢性萎缩性胃炎(CAG)是高发消化系统疾病，以胃固有腺体萎缩，常伴有肠上皮化生、上皮内瘤变为核心病理特征，属于明确的胃癌前情况。西医缺乏逆转腺体萎缩的特效方案，仅可根除幽门螺杆菌(Hp)、对症改善症状，难以阻断病变长期进展。胃复春作为《慢性萎缩性胃炎诊疗指南(2022版)》推荐中成药，凭借健脾益气、解毒化瘀、行气和胃功效，实现多靶点干预CAG病理进程。近年网络药理学、循证医学研究逐步阐明其作用通路，单药及联合西药方案已形成分层诊疗体系。本文从组方理论、现代药理机制、临床分层应用、用药安全性、研究短板及未来方向展开系统综述，为基层临床规范化用药、中医药干预胃癌前病变科研设计提供理论依据与实践参考。
Abstract: Chronic atrophic gastritis (CAG) is a highly prevalent digestive-system disease, whose core pathological features include atrophy of gastric intrinsic glands, intestinal metaplasia and intraepithelial neoplasia, and it is a recognized precancerous lesion of gastric cancer. Western medicine lacks specific therapies to reverse gland atrophy; current interventions mainly consist of Helicobacter pylori (Hp) eradication and symptomatic supportive treatment, which have limited efficacy in halting the long-term progression of precancerous gastric mucosal lesions. As a recommended Chinese patent medicine in the 2022 Guidelines for the Diagnosis and Treatment of Chronic Atrophic Gastritis, Weifuchun exerts the effects of invigorating the spleen and replenishing qi, detoxifying and resolving stasis, as well as activating qi and harmonizing the stomach, and intervenes in the pathological evolution of CAG via multi-target pathways. In recent years, network pharmacology and evidence-based medicine have continuously uncovered its molecular action pathways, and stratified therapeutic regimens of Weifuchun as monotherapy or combined with western medicines have been gradually established. This review summarizes the formula-based theory, modern pharmacological mechanisms, stratified clinical applications, medication safety, existing research deficiencies and future research directions of Weifuchun, so as to provide theoretical basis and practical references for standardized clinical medication at primary-care settings and research design of traditional Chinese medicine intervening in gastric precancerous lesions.
