瑞马唑仑与环泊酚在全身麻醉中的应用进展

摘要: 瑞马唑仑和环泊酚是近年来静脉麻醉领域的两类新型药物,在全身麻醉中展现出显著的临床优势。瑞马唑仑是一种超短效苯二氮䓬类药物,通过非特异性酯酶迅速水解代谢,具有起效快、恢复迅速、无蓄积、血流动力学稳定等特点;环泊酚则是我国首个自主研发的I类创新静脉麻醉药,作为丙泊酚的结构衍生物,其与GABA_A受体的亲和力约为丙泊酚的4~5倍,在较低剂量下即可达到等效镇静效果,且注射痛发生率极低、呼吸循环抑制轻。本文从药理学特性、全身麻醉诱导与维持、特殊人群应用、与经典药物的对比以及两种药物的联合应用等角度,系统综述瑞马唑仑与环泊酚在全身麻醉中的研究进展,以期为临床麻醉用药提供参考。
Abstract: Remimazolam and ciprofol are two novel intravenous anesthetics that have demonstrated significant clinical advantages in general anesthesia in recent years. Remimazolam is an ultra-short-acting benzodiazepine that is rapidly hydrolyzed and metabolized by non-specific esterases, characterized by rapid onset, rapid recovery, no accumulation, and stable hemodynamics. Ciprofol is the first Class I innovative intravenous anesthetic independently developed in China. As a structural derivative of propofol, it has an affinity for the GABA_A receptor approximately 4 to 5 times that of propofol, can achieve an equivalent sedative effect at lower doses, and is associated with a very low incidence of injection pain and mild respiratory and circulatory depression. This article systematically reviews the research progress of remimazolam and ciprofol in general anesthesia from the perspectives of pharmacological properties, induction and maintenance of general anesthesia, application in special populations, comparison with classic drugs, and the combined use of the two drugs, in order to provide a reference for clinical anesthesia medication.
