Abstract: In the field of assisted reproductive technology, with the delay in female childbearing age, the rate of embryonic chromosomal aneuploidy is significantly increased in women of advanced maternal age. Although preimplantation genetic testing for aneuploidy (PGT-A) can screen for euploid embryos, the proportion of cycles yielding at least one euploid embryo remains low in this population. Growth hormone (GH), a key endocrine regulator, has recently been shown to potentially improve oocyte mitochondrial function and chromosome segregation during meiosis, thereby enhancing the euploid embryo acquisition rate in PGT-A cycles among advanced-age women. This article systematically reviews the current application, mechanisms of action, and effects of GH on embryo quality and pregnancy outcomes in PGT-A cycles for advanced-age women, aiming to provide scientific evidence for clinical practice and promote individualized assisted reproductive therapy.
