Zhao Zhengyu’s Experience in Treating PMOS-Related Infertility Using the “Tiaozhou Shiliang” Acupuncture Method
摘要: 文章总结赵征宇教授运用“调周施量”针法治疗多内分泌代谢性卵巢综合征(Polyendocrine Metabolic Ovarian Syndrome,PMOS,原称多囊卵巢综合征)相关不孕症的临床经验。赵教授认为,本病以肾虚、冲任失调为本,兼夹脾虚痰湿、肝郁血瘀及心神失调。治疗以月经周期阴阳消长为纲,依据经血、带下、基础体温及超声卵泡发育等征象辨识生殖阶段,形成“周以象定、法以证立、量随周调”的诊疗思路。在基础方上随证加减,并根据卵泡蓄养、排卵发动、胞宫藏纳及行经开泄等阶段,动态调整针刺强弱、留针时间、治疗频次及灸法。该法突出辨周、辨证与辨量相结合,可为PMOS相关不孕症的针灸分期治疗提供参考。
Abstract: This paper summarizes Zhao Zhengyu’s clinical experience in treating infertility associated with polyendocrine metabolic ovarian syndrome (PMOS), formerly known as polycystic ovary syndrome (PCOS), using the “Tiaozhou Shiliang” acupuncture method. Zhao attributes the underlying pathogenesis of the condition to kidney deficiency and dysregulation of the Chong and Ren channels, often accompanied by spleen deficiency with phlegm-dampness, liver qi stagnation with blood stasis, and heart-spirit disturbance. Treatment is guided by the cyclical waxing and waning of yin and yang throughout the menstrual cycle. Reproductive phases are identified based on menstrual characteristics, vaginal discharge, basal body temperature, and ultrasonographic findings of follicular development. This approach follows the principles of “identifying the cycle based on manifestations, establishing treatment according to syndrome differentiation, and adjusting stimulation dose according to the cycle”. A core acupoint prescription is modified according to the individual syndrome pattern, while acupuncture stimulation intensity, needle retention time, treatment frequency, and the use of moxibustion are dynamically adjusted across different reproductive phases, including follicular development, ovulation, the postovulatory phase, and menstruation. By integrating cycle differentiation, syndrome differentiation, and dose adjustment, the “Tiaozhou Shiliang” acupuncture method may inform phase-specific acupuncture treatment for PMOS-related infertility.
