Advances in Application and Safety of JAK Inhibitors in Precision Therapy for Rheumatic Immune Diseases
Abstract: As a novel class of small-molecule targeted therapies, Janus kinase (JAK) inhibitors have demonstrated significant efficacy in the treatment of rheumatic immune diseases by blocking the JAK-STAT signaling pathway. This review aims to systematically summarize the application status and safety research progress of JAK inhibitors in the precision treatment of rheumatic immune diseases. The article first delves into the pathogenic mechanism of the JAK-STAT signaling pathway in rheumatic immune diseases, analyzing the differences in receptor selectivity between first-generation and second-generation JAK inhibitors and the implications of their pharmacokinetic characteristics for precision medicine. Subsequently, it elaborates on the clinical benefits and efficacy differences of JAK inhibitors in diseases such as rheumatoid arthritis, spondyloarthritis, and systemic lupus erythematosus, with a critical analysis of the mechanisms underlying efficacy heterogeneity across different disease contexts. Finally, it focuses on safety issues such as infection risk, cardiovascular adverse events, and malignancy risk, establishing a dedicated mechanistic discussion on the controversial evidence regarding cardiovascular risk, and summarizing corresponding laboratory monitoring strategies. Current evidence suggests that JAK inhibitors have definite efficacy, but their safety risks need to be balanced through precise patient selection and strict monitoring management. Future research should focus on further optimizing drug selectivity, and promoting truly individualized treatment through biomarker-guided prospective head-to-head trials.
