摘要: 目的:针对当日多名患者连续接受介入手术的实际情况,构建用于整合护理交接关键信息的管理总表,并对原有交接流程进行调整,在此基础上评价其在心血管内科中的应用情况。方法:将分散在不同信息系统中的护理交接相关信息进行汇总,设计“当日介入手术总表”,并结合临床工作流程对交接方式进行调整,使当日介入手术患者信息能够集中呈现,形成“一表总览、一单交接”的交接模式。在此基础上,采用历史对照研究方法,比较实施前后交接相关流程延误事件发生率、系统切换次数、单例患者交接时间及护理交接模式满意度。结果:实施后,与原有方式相比,交接相关流程延误事件发生率降低、系统切换次数减少、单例患者交接时间缩短、护理交接模式满意度明显提高,差异均有统计学意义(P < 0.05)。结论:通过对分散信息进行集中呈现并对交接流程加以调整,可以减少信息系统切换次数,缩短交接时间,在一定程度上改善护理人员的使用体验。
Abstract: Objective: In response to the common scenario in which multiple patients undergo interventional procedures on the same day, a centralized summary table was developed to integrate key information required for nursing handover. The existing handover workflow was subsequently adjusted, and its application in a cardiology setting was evaluated. Methods: Information related to nursing handover, originally scattered across different information systems, was consolidated to develop a “daily interventional procedure summary table”. Based on this, the handover workflow was adjusted in line with clinical practice, enabling centralized presentation of patient information and establishing a model characterized by “one-sheet overview and single-form handover”. On this basis, a historical controlled study design was adopted to compare the incidence of handover-related process delays, the frequency of system switching, the time required for handover per patient, and nurses’ satisfaction with the handover model before and after implementation. Results: After implementation, compared with the previous approach, the incidence of handover-related process delays decreased, the frequency of system switching was reduced, the time required for handover per patient was shortened, and nurses’ satisfaction with the handover model increased; all differences were statistically significant (P < 0.05). Conclusion: By consolidating dispersed information into a centralized display and adjusting the handover workflow, the frequency of system switching can be reduced and the time required for handover can be shortened, thereby improving nurses’ user experience to some extent.
