Abstract: Objective: To evaluate the efficacy of 5-fluorouracil (5-FU) combined with interferon-alpha (IFN-α) in eradicating genital human papilloma virus (HPV) infection, and to explore the dosage, administration mode, treatment course as well as the synergistic mechanism of the combined regimen. Methods: A total of 214 patients diagnosed with positive high-risk HPV in the Clinical Laboratory of our hospital from 2022 to 2025 were enrolled. All patients had normal TCT results, and cervical and vaginal intraepithelial neoplasia was excluded by biopsy under colposcopy. According to patients’ willingness, 81 patients were assigned to the blank control group receiving regular observation only; the rest received drug treatment and were divided into three subgroups: 88 cases in the interferon monotherapy group (treated with interferon for 3 months), 21 cases in the 5-FU monotherapy group, and 24 cases in the combined 5-FU plus interferon group. Reexaminations were performed at 3 months and 6 months after drug withdrawal. Local irritation reactions during medication were recorded. The HPV negative conversion rate, incidence of local adverse reactions and drug tolerance were compared among the four groups. Results: After 3 months of observation or treatment, the HPV negative conversion rate was 39.5% in the blank control group, 44.3% in the interferon treatment group, 61.9% in the 5-fluorouracil (5-FU) treatment group, and 62.5% in the combined treatment group. The overall difference in the 3-month negative conversion rates among the four groups was statistically significant (χ2 = 9.62, P = 0.022 < 0.05). At 6 months following the completion of observation or treatment, the negative conversion rates of the four groups were 39.5%, 47.7%, 66.7% and 70.8%, respectively; chi-square test demonstrated a statistically significant intergroup difference (χ2 = 27.06, P < 0.001). No statistically significant difference was observed in HPV negative conversion rates between the blank control group and the interferon group (39.5% vs 47.7%, χ2 = 1.158 < 3.841, P > 0.05). Likewise, there was no significant difference between the interferon group and the combined treatment group (χ2 = 2.497 < 3.841, P > 0.05). The incidence of adverse reactions (combining mild and moderate-to-severe adverse events) across the four groups was 0%, 6.17%, 42.09% and 45.08%, respectively, with statistically significant differences (P < 0.05). In addition, the 6-month negative conversion rates were 66.7% in the 5-FU monotherapy group and 70.8% in the combined regimen group (5-FU plus interferon), and the difference between the two groups was not statistically significant (χ2 = 0.091 < 3.841, P > 0.05). Conclusion: Results of this small-sample study revealed that topical 1% low-concentration 5-FU yielded HPV negative conversion rates of 61.9% at 3 months and 66.7% at 6 months for genital epithelial HPV infection, presenting satisfactory therapeutic efficacy. The curative effect was numerically superior to that of interferon monotherapy (44.3% at 3 months, 47.7% at 6 months), yet no statistically significant difference was identified, which may be attributed to the limited sample size of the 5-FU group. Further studies with enlarged sample sizes are required to draw more reliable conclusions.
