Abstract: Objective: Chemotherapy-Related Cognitive Impairment (CRCI) in breast cancer significantly impacts patients’ quality of life and has become a research topic of growing concern in the field of oncology nursing. However, the current research landscape, hotspots, and trends in this area lack systematic visualization. This study aims to conduct a visual analysis of CRCI research in breast cancer using CiteSpace software, to understand its research status, hotspots, and trends, and to provide new insights for future nursing practice. Methods: Relevant literature on breast cancer CRCI was retrieved from the China National Knowledge Infrastructure (CNKI) and Web of Science (WOS) databases for the period from January 1, 2000 to October 31, 2025. A visualization analysis was performed to review the annual publication trends, summarize the collaborative network characteristics of authors and institutions, and analyze the distribution of research themes reflected by keyword co-occurrence and clustering. Results: A total of 141 Chinese articles from CNKI and 517 English articles from WOS were included. The annual publication volume in this field exhibited a cyclical fluctuation trend. Internationally, stable collaborative teams have been formed, with relatively close inter-institutional cooperation. Chemotherapy was identified as the most critical influencing factor, while cognitive impairment and memory decline were the most common clinical manifestations of CRCI. Conclusion: Chemotherapy remains the core influencing factor, with cognitive impairment, memory decline, and quality of life being the most common clinical presentations and outcome indicators. While stable research collaboration networks have been established internationally—especially in North America and Europe—domestic institutional and author collaboration networks in China remain sparse, and a stable core author group has yet to be formed. Cross-regional collaborative networks remain weak. Based on these findings, this study proposes that future research should further strengthen in-depth collaboration across countries, institutions, and scholars. The analysis suggests that non-pharmacological interventions, cognitive training, psychosocial support, and personalized care models will become the research frontiers and hotspots in this field, providing a clear direction for clinical nursing practice and future research on breast cancer CRCI.
