Liposomal Bupivacaine for Postoperative Analgesia Following Peripheral Nerve Block: An Evidence Synthesis in Which the Comparator Determines the Clinical Conclusion
Abstract: Liposomal bupivacaine (LB) is intended to prolong postoperative analgesia with a single injection, yet its efficacy conclusions are inconsistent across procedures and studies. The key to this inconsistency lies not only in the formulation itself, but also in differences in comparators, block sites, background multimodal analgesia regimens, and outcome definitions. Based on formulation studies, randomized controlled trials, systematic reviews, safety data, and analgesic-outcome methodology literature purposively collected around this question, this narrative synthesis addresses the following question: relative to which regimen, in which procedure, can LB yield a clinically worthwhile incremental net benefit? Current evidence indicates that LB has definite analgesic activity versus placebo; however, its incremental benefit is inconsistent when compared with clinically relevant active controls such as plain local anesthetics, local anesthetics plus dexamethasone, or continuous catheters. Popliteal sciatic nerve block for foot and ankle surgery, particularly bunionectomy, currently provides the most persuasive evidence of concurrent improvement in pain and opioid consumption. In shoulder surgery, effect signals exist, but the results depend strongly on the comparator, and phrenic nerve and pulmonary effects must be taken seriously. Studies of periarticular infiltration in total knee arthroplasty and of a specific parasternal plane block in cardiac surgery do not support routine use. The clinical positioning of LB should not be built on a presumed “long-acting” duration, but should be judged jointly by integrated pain-opioid outcomes, quality of recovery, respiratory and motor safety, catheter management burden, and cost. We recommend positioning LB as a selective tool within multimodal analgesia rather than a routine replacement for all peripheral nerve blocks.
