Construction and Evidence Boundary of the “Heat-Turbidity-Stasis-Toxin-Collateral Damage” Pathogenesis Model of Diabetic Kidney Disease Based on Liu Wansu’s Theory of Fire-Heat
Abstract: Objective: To reinterpret the dynamic role of excess pathogenic factors in diabetic kidney disease (DKD) based on Liu Wansu’s theory of fire-heat, to construct a pathogenesis model of “heat-turbidity-stasis-toxin-collateral damage”, and to clarify the boundary of its clinical application. Methods: This narrative theoretical study took classical medical texts such as Suwen Xuanji Yuanbingshi and Sanxiao Lun as theoretical sources, and comprehensively analyzed the pathogenesis evolution, syndrome stratification, treatment configuration and evidence level of DKD by integrating recent integrated Chinese and Western medicine guidelines, studies on TCM syndrome distribution, pooled analyses of randomized controlled trials, systematic reviews and experimental pharmacological studies. Results: DKD should not be interpreted as a purely deficiency-related disorder. Qi-yin deficiency and spleen-kidney deficiency constitute the root, while fire-heat, damp turbidity, blood stasis and turbid toxin may participate in disease progression at different stages. The proposed model can be summarized as “fire-heat damaging fluids, fluid depletion generating turbidity, heat and stasis binding together, stasis-toxin damaging collaterals, and failure of qi transformation”. Current syndrome studies and guidelines support the view of root deficiency with branch excess, but are insufficient to establish a fixed one-to-one correspondence between TCM syndromes and modern DKD stages. Clinical evidence suggests that TCM combined with conventional Western medicine may have potential benefits in reducing proteinuria and improving some renal function indices, but methodological limitations and insufficient long-term outcomes restrict the strength of conclusions. Conclusion: The value of interpreting DKD through Liu Wansu’s theory of fire-heat lies in suggesting that syndrome differentiation and treatment should not lean toward exclusive tonification, but should dynamically adjust the emphasis of heat-clearing, turbidity-resolving, blood-activating, detoxifying and collateral-unblocking methods together with qi-yin nourishment and spleen-kidney reinforcement according to syndrome manifestations and renal indices. This model currently mainly serves theoretical integration and clinical heuristic purposes, and still needs to be verified through syndrome stratification, multicenter randomized controlled trials and long-term follow-up outcomes.
