ICU医护人员肠内营养目标管理体验的质性研究
A Qualitative Study on the Experience of Enteral Nutrition Target Management among ICU Healthcare Workers

摘要: 目的：了解医护人员进行肠内营养目标管理的促进及阻碍因素，探讨医护人员对肠内营养目标管理的体验，为以后的肠内营养目标管理方案提供参考和依据。方法：采用目的抽样的方法，选取2023年12月~2024年1月重庆市某五家三级甲等医院ICU工作的11名护士及2名医生进行半结构式访谈，运用Colaizzi 7步分析法对访谈资料进行整理。结果：提炼出3个主题及所属的11个亚主题：肠内营养目标管理的阻碍因素(医护人员对肠内营养的重视度及认知不足、医护原因导致的肠内营养中断时间持久、患者本身疾病复杂或胃肠不耐受、ICU患者转出后肠内营养方案不连续)、肠内营养目标管理的促进因素(对ICU患者肠内营养进行精细化管理、护士动态评估肠内营养实施效果、及时且有效地处理肠内营养的相关并发症)、肠内营养目标管理的建议(增加简单有效的肠内营养相关培训、加强医护间的沟通，促进多学科合作、肠内营养加温、更新设备或应用新技术)。结论：医护人员在进行肠内营养目标管理过程中存有障碍及促进因素，应通过规范肠内营养管理流程提高医护人员的肠内营养管理意识，以更好地管理ICU患者的肠内营养。
Abstract: Objective: To understand the facilitating and hindering factors for healthcare workers to perform enteral nutrition goal management, and to explore healthcare workers’ experience of enteral nutrition goal management, so as to provide reference and basis for future enteral nutrition goal management programmes. Methods: Using purposive sampling, 11 nurses and 2 doctors working in the ICU of a five tertiary hospital in Chongqing from December 2023 to January 2024 were selected for semi-structured interviews, and the Colaizzi 7-step analysis method was used to collate the interview data. Results: Three themes and 11 sub-themes were extracted: obstacles to enteral nutrition target management(insufficient attention and knowledge of healthcare personnel to enteral nutrition, prolonged interruption of enteral nutrition due to healthcare reasons, complicated diseases or gastrointestinal intolerance of patients themselves, discontinuity of enteral nutrition programme after transferring out of the ICU), facilitators to enteral nutrition target management (meticulous management of enteral nutrition for ICU patients, dynamic assessment of the implementation effect by nurses, timely and effective management of complications related to enteral nutrition), and recommendations (increase of training related to simple and effective enteral nutrition, strengthening of inter-medical assessment of the implementation effect, and timely and effective management of complications related to enteral nutrition). (fine management of enteral nutrition in ICU patients, dynamic assessment of the effect of enteral nutrition implementation by nurses, timely and effective management of complications related to enteral nutrition), and suggestions for the target management of enteral nutrition (increasing simple and effective training related to enteral nutrition, strengthening communication between healthcare professionals and nurses, promoting multidisciplinary cooperation, warming enteral nutrition, updating equipment, or applying new technologies). Conclusion: There are barriers and facilitators in the process of enteral nutrition target management by healthcare professionals, and the awareness of enteral nutrition management by healthcare professionals should be improved by standardising the process of enteral nutrition management in order to better manage enteral nutrition in ICU patients.
