Abstract: Sacroiliac joint-related pain is an important source of nonspecific low back pain. In traditional Chinese spinal manipulation literature, sacroiliac joint “malposition” is used to describe a functional imbalance and should be differentiated from traumatic dislocation. Repositioning manipulation is a commonly used nonpharmacological intervention. Rather than seeking a simple immediate anatomical repositioning, it works within a traditional Chinese medicine health-management pathway that combines release of local fascia and relaxation of tense muscles, directional manipulation, lumbopelvic motor-control retraining, patient education, and home exercise to promote pain relief and functional recovery. Clinical studies in sacroiliac joint dysfunction directly support possible improvements in pain and function after manual interventions. Evidence for changes in peripheral sensory input, spinal-supraspinal pain modulation, and proprioceptive postural control is largely indirect, deriving from studies of other spinal manual techniques or chronic low back pain. This article therefore proposes an “peripheral stimulation-central integration-motor output-functional improvement” model; its causal links remain to be tested specifically for sacroiliac repositioning manipulation. Resting-state functional magnetic resonance imaging may serve as an exploratory observation tool. Clinical use should include strict screening for contraindications, standardized documentation of key procedural details and adverse events, and integration with staged rehabilitation and recurrence prevention.
