寒地龙医视域下龙江医派骨伤科传承体系构建与经典普及创新研究

Research on the Construction of Inheritance System and Innovation of Classical Popularization for Orthopedics and Traumatology of Longjiang Medical School from the Perspective of Cold-Earth Dragon Medicine

摘要: 寒地龙医是龙江医派区别于国内其他骨伤流派的核心学术标识。既往研究多聚焦龙江医派整体理论梳理，针对骨伤专科分支缺少质性调研支撑，少数民族正骨技艺源流考证薄弱，传承方案趋于通用范式，缺少适配东北高寒冰雪地域的区域化设计。本文基于寒地龙医理论，总结寒地骨病“寒凝–血瘀–湿阻–气虚–骨损”递进病机假说，对龙江骨伤“形气辨证”予以规范化概念释义，采用文献研究结合后续拟开展质性研究方案、典型病案例证的模式，梳理龙江医派骨伤科学术脉络，考证肃慎–女真少数民族冰雪疗伤技艺与中原骨伤理论的融合关系，剖析流派传承现实困境；构建“史料抢救–理论重构–技术标准化–人才梯度培育–冰雪场景成果转化”五位一体专属传承体系，并形成“寒地龙医·冰雪筋骨养护”可落地科普IP实施方案。结果表明：龙江医派骨伤科以寒瘀为本、形气同调、内外合治为核心学术特征；当前除学术碎片化之外，尚存在少数民族正骨口述史料流失、寒地筋骨病证及冰雪运动相关筋骨损伤本土诊疗经验未系统固化、寒地外用膏剂受药材、工艺及审批多重制约产业化受阻等现实问题。本研究形成的病机假说、本土概念释义、少数民族技艺源流梳理以及面向冰雪地域的传承–科普一体化方案，可为寒地中医骨伤活态传承、寒地筋骨病证、冰雪运动相关筋骨损伤的中医药防控提供理论参考与实践范式。

Abstract: Cold-Earth Dragon Medicine represents the core academic hallmark that distinguishes the Longjiang Medical School from other domestic traumatology schools. Previous studies have mostly focused on sorting out the overall theories of the Longjiang Medical School, yet they lack qualitative research evidence for its orthopedics-traumatology subdivision. The origin of ethnic minority bone-setting techniques remains poorly documented, existing inheritance schemes follow generic paradigms, and region-specific frameworks tailored to the frigid, ice-and-snow-prone terrain of Northeast China are scarce. Based on the theories of Cold-Earth Dragon Medicine, this paper summarizes a progressive pathogenesis hypothesis for cold-region bone disorders: cold congelation-blood stasis-damp obstruction-qi deficiency-bone impairment. It further standardizes the conceptual definition of “form-qi syndrome differentiation” in Longjiang traumatology. Adopting a combined approach of literature review, a design for qualitative research planned for a later stage and typical case illustration, this study traces the academic lineage of orthopedics and traumatology within the Longjiang Medical School, investigates the integration between ice-snow-related healing techniques of the Sushen-Jurchen ethnic minorities and the orthopedic theories of the Central Plains, and analyzes practical dilemmas in school inheritance. A five-in-one specialized inheritance system is constructed, covering historical-document rescue, theoretical reconstruction, technical standardization, tiered talent cultivation, and achievement transformation for ice-snow scenarios. A feasible implementation plan is also developed for the popular-science IP “Cold-Earth Dragon Medicine · Ice-Snow Musculoskeletal Care”. The results demonstrate that orthopedics and traumatology of the Longjiang Medical School features cold-stasis as its fundamental pathogenesis, concurrent regulation of form and qi, and combined internal-external therapy. Apart from academic fragmentation, prominent practical challenges include the loss of oral historical records on ethnic-minority bone-setting, insufficient systematic documentation of local clinical experience for cold-region musculoskeletal disorders and ice-snow-sports-related musculoskeletal injuries, and barriers hindering the industrialization of topical cold-region plasters due to constraints in medicinal materials, manufacturing processes and regulatory approval. The pathogenesis hypothesis, standardized local-concept interpretation, origin analysis of ethnic-minority therapeutic techniques, and integrated inheritance-popular-science framework proposed in this research can provide theoretical references and practical models for the living inheritance of cold-region TCM orthopedics-traumatology, as well as for TCM-based prevention and management of cold-region musculoskeletal diseases and ice-snow-sports-associated musculoskeletal injuries.
