Abstract: Subthreshold depression (SD) is a subclinical state between mental health and major depressive disorder, characterized by insidious symptoms, high prevalence, and a high likelihood of progressing to clinical depression. Recent studies have shown that individuals with SD exhibit abnormal emotional processing, primarily characterized by negative attentional bias and reduced processing of positive stimuli. Eye-tracking technology can record an individual’s visual attention allocation process in real time, providing a new research tool for revealing the attentional bias and emotional processing characteristics of SD. This article systematically reviews the progress of eye-tracking research on emotional processing in SD, focusing on eye-tracking experimental paradigms, attentional bias characteristics, emotional processing abnormalities, and their application in SD identification. It also analyzes the limitations of current research and future development directions. Existing research indicates that eye-tracking features have good application potential in the auxiliary identification and objective assessment of SD, but their clinical application still requires further advancements in research paradigm standardization, multimodal fusion, and clinical validation. This review can provide a theoretical basis for the early identification, objective assessment, and intelligent screening of SD, and offer a reference for related research and clinical applications.
