Abstract: Objective: To measure the angles between the semicircular canals and their angles relative to the reference horizontal plane, and to determine whether variations in semicircular canal angles are associated with the onset of BPPV. Methods: In this study, 30 patients with primary BPPV and 30 healthy individuals were selected from Guilin Hospital of Traditional Chinese Medicine between 2023 and 2025. The angles between the semicircular canals and the angles between each semicircular canal and the reference horizontal plane were measured using the 3D-FIESTA sequence of magnetic resonance imaging, and the differences between the two groups were compared. Results: The human semicircular canals were not completely perpendicular to each other. The mean angle among the three semicircular canals was 92.3˚, with a maximum angle of 116.6˚ and a minimum angle of 66.8˚. In the observation group, the angles of right anterior - right posterior, left anterior - left posterior, bilateral horizontal semicircular canal - reference horizontal plane, and left posterior semicircular canal - reference horizontal plane were all significantly different from those in the control group (P < 0.05). Conclusion: The human semicircular canals are not completely perpendicular to each other, and variations in semicircular canal angles exist among different individuals. Some inter-semicircular canal angles in BPPV patients differ significantly from those in the control group, and such anatomical variations may be related to the occurrence of BPPV.
