Abstract: The systemic immune-inflammation index (SII) is a composite inflammatory indicator derived from peripheral blood neutrophil, platelet, and lymphocyte counts, which comprehensively reflects the host’s systemic inflammatory status and the imbalance between systemic inflammation and antitumor immune function. In recent years, accumulating evidence has indicated that a high pretreatment SII is closely associated with poor prognosis in respiratory system malignancies, particularly non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC). Based on a systematic literature search, this review summarizes recent advances in the research of SII in respiratory system tumors, focusing on its biological basis, prognostic value across different stages of lung cancer, predictive value in immunotherapy, and application in special populations. Overall, as a low-cost, simple, non-invasive, and readily accessible prognostic marker, SII shows promising prospects for risk stratification and individualized treatment decision-making in respiratory system tumors, and is expected to become a practical biomarker for guiding individualized therapy.
