用焦虑-VAS评估音乐疗法对白内障手术老人焦虑的作用

摘要: 目的:探讨术中音乐疗法对白内障手术老人焦虑的作用。方法:白内障手术老人患者40例,随机分为实验和对照组,每组各20例,实验组术中接受音乐疗法,对照组进行常规护理。采用焦虑-VAS分别于术前和术毕评估焦虑程度,同时记录血压、心率以及二者之积。结果:术前实验和对照组患者焦虑-VAS分别为7.2 ± 2.0和7.3 ± 1.9,差异无统计学意义(p > 0.05)。实验组术毕焦虑-VAS为4.8 ± 1.8,显著低于术前水平,差异有统计学意义(p < 0.01),对照组术毕为7.5 ± 2.1;对照组术毕焦虑-VAS与术前水平比,差异无统计学意义(p > 0.05);两组比较,实验组术毕焦虑-VAS显著低于对照组水平,差异有统计学意义(p < 0.01)。术前两组比较患者收缩压、心率以及心率–血压乘积无明显不同,差异无统计学意义(p > 0.05),手术10分钟和术毕各参数显著增加,差异都有统计学意义(p < 0.05),但实验组术中和术毕各参数增加程度小于对照组,差异无统计学意义(p < 0.05)。结论:白内障手术中音乐疗法可缓解手术患者焦虑水平。
Abstract: Objective: To investigate the effect of intraoperative music interventions on the anxiety of elderly patients with cataract surgery. Methods: 40 elderly patients with cataract surgery were randomly divided into study and control groups (n = 20). The study group received music interventions during operation and the control group received routine nursing. Anxiety was assessed with visual analogue scales (anxiety-VAS) before and after operation, respectively. Meanwhile blood pressure, heart rate and the pressure-rate product were recorded. Results: The anxiety-VAS scores were 7.2 ± 2.0 and 7.3 ± 1.9, respectively, at preoperation in study and control groups with no significant difference (p > 0.05). The anxiety-VAS was 4.8 ± 1.8 at end of operation, which was significantly lower than preoperative level and the difference was statistically significant (p < 0.01) in study group. The anxiety-VAS was 7.5 ± 2.1, compared with preoperative level with no significant difference (p > 0.05) in the control group. The anxiety-VAS was significantly lower at end of operation in study than control group (p < 0.01). There were no significant differences in systolic blood pressure, heart rate and pressure-rate product before operation between the two groups (p> 0.05), and those parameters were significantly increased (p < 0.05 ) at 10 min and end of operation. But there was no significant difference between the two groups in the intraoperative and postoperative parameters; the difference was not statistically significant (p > 0.05). The pressure, heart rate and pressure-rate products were significantly lower in study than control group (p < 0.01). Conclusion: Music interventions could relieve the anxiety in patients with cataract surgery.
