畲药龙斑汤联合内镜干预逆转胃癌前病变的临床应用

Clinical Application of She Medicine Longban Decoction Combined with Endoscopic Intervention to Reverse Gastric Precancerous Lesions

摘要: 目的：观察畲药龙斑汤联合内镜干预逆转胃癌前病变的临床效果。方法：选择胃黏膜病变，组织学显示为黏膜低级别或高级别上皮内瘤变共98例，根据病变的大小、性质通过内镜下APC、EMR、ESD治疗，术后口服畲药龙斑汤。结果：APC共治疗60次，包括3次及以上治疗患者2例，2次治疗患者6例，1次治疗完成42例；EMR及ESD治疗均为一次性完成。98例患者均完成随访，在末次治疗术后1月复查“人工溃疡”均已愈合，术后1、3、6、12个月复查均无复发，本组中未出现出血、穿孔等并发症。结论：内镜治疗消除胃黏膜隆起性病变效果显著，畲药龙斑汤治疗“人工溃疡”效果较为理想，值得临床推广应用。

Abstract: Objective: To observe the clinical effect of she medicine longban decoction combined with endo-scopic intervention in the reversal of gastric precancerous lesions. Methods: A total of 98 cases were included, with histopathology of low-grade or high-grade intraepithelial neoplasia. Endoscopic APC, EMR or ESD was adopted according to the size and nature of the lesion, then she medicine dragon soup was administrated postoperatively. Results: APC was treated 60 times, including 2 patients with 3 or more treatments, 6 patients with 2 treatments and 42 patients with 1 treatment. Both EMR and ESD treatment were one-time. All of 98 patients completed follow-up. When followed-up in the first month after the last treatment, the “artificial ulcer” has healed. There was no recurrence after 1, 3, 6, 12 months of follow-up. No complications such as hemorrhage or perforation occurred in this group. Conclusion: Endoscopic treatment has an ideal effect on the elimination of gastric mucosal uplift venereal disease, and she longban decoction has an ideal effect on the treatment of “artificial ulcer”, which is worthy of clinical application.
