直肠癌术前高分辨率MR分期的准确性研究

摘要: 目的:使用高分辨率磁共振探讨直肠癌术前MR分期与术后病理分期对照的准确性。方法:选取2017年1月至2018年5月期间在南昌大学第一附属医院手术病理确诊为直肠癌,术前一周内行MRI检查的患者47例,回顾性对照研究术前高分辨率MR分期的准确性。结果:T分期准确率87.2%,Kappa值0.779 (P < 0.05);N分期准确率78.7%,Kappa值0.543 (P < 0.05)。结论:高分辨率MR在直肠癌术前TN分期中的准确率高,多方位多序列对直肠癌进行观察可以提高分期的准确率。
Abstract: Objective: To evaluate the accuracy of MRI staging in rectal cancer with high-resolution magnetic resonance imaging (HR-MRI). Method: From January 2017 to May 2018, 47 patients with rectal cancer diagnosed by operation and pathology in the first affiliated Hospital of Nanchang University were examined with MRI within one week before operation. The accuracy of preoperative high resolution MR-staging was studied retrospectively. Result: T staging accuracy of 87.2%, kappa = 0.779 (P < 0.05); N staging accuracy of 78.7%, kappa = 0.543 (P < 0.05). Conclusion: HR-MRI can accurately predict T staging and N staging of rectal cancer preoperatively, and comprehensive analysis of multi-orientation and multi-sequence can improve the accuracy of staging.
