个性化护理干预对体外循环术后急性肾损伤转归的影响

Effect of Individualized Nursing Intervention on Acute Kidney Injury after Cardiopulmonary Bypass

摘要: 目的：探讨个性化护理对体外循环术后急性肾损伤转归的影响。方法：收录患者26例，随机分为常规护理组和个体化干预护理组，分别给予常规护理和个体化干预护理，评价不同护理的结果差异。结果：术后第5及第10天血肌酐值进行对比，个体化干预护理后均下降明显(P < 0.05)；个体化干预护理组监护室时间、停静脉利尿剂时间、转透析治疗均优于常规组(P < 0.05)。结论：采用个性化护理措施，包括个体化观察生命体征，监测尿量变化，加强液体管理，改善氧疗，避免加重肾功能损害的药物，个体化维持水电解质平衡，预防感染等，可有效缩短体外循环术后急性肾损伤的恢复时间，改善急性肾损伤的转归。

Abstract: Objective: To explore the effect of personalized nursing on the prognosis of acute kidney injury after cardiopulmonary bypass. Methods: 26 patients were randomly divided into routine nursing group and individualized nursing group, which were given routine nursing and individualized in-tervention nursing to evaluate the difference of the results of different nursing. Results: The values of blood creatinine were compared between fifth and 10th day after the operation, and the in-dividual intervention nursing decreased significantly (P < 0.05), and the ICU time, no-diuretic time, and dialytic treatment in the individualized intervention nursing group were all better than those of the conventional group (P < 0.05). Conclusion: Individualized nursing measures, including indi-vidualized observation of vital signs, monitoring of changes in urine volume, strengthening liquid management, improving oxygen therapy, avoiding drugs that aggravate renal impairment, indi-vidualized maintenance of water and electrolyte balance and preventing infection, can effectively shorten the recovery time of acute kidney injury after cardiopulmonary bypass and improve the outcome of acute kidney injury.
