甲状腺功能正常中青年血清促甲状腺激素水平与代谢综合征的相关性研究
The Study on the Correlation between Serum Levels of Thyroid Stimulating Hormone and Metabolic Syndrome in Euthyroid Young Adults
摘要: 目的:探讨甲状腺功能正常的中青年人群血清促甲状腺激素水平(TSH)与代谢综合征(MS)及其各组分的关系。方法:采用回顾性研究方法,收集2016年9月至2017年9月就诊于宁夏医科大学心脑血管病医院体检办、年龄在20~60岁之间共2576人,其中甲状腺功能正常中青年687人,合并有MS的患者396人(包括男性291人,女性105人),平均年龄(47.68 ± 8.83岁)。收集研究对象一般资料,包括:姓名、性别、年龄、身高、体重、体重指数(BMI)、空腹血糖(FPG)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、收缩压、舒张压及血清TSH,分析甲状腺功能正常中青年血清TSH水平与MS及其各组分的相关性。结果:宁夏地区中青年甲状腺功能正常且合并MS的发生率为15.37% (396/2576)。将TSH分为高值组及低值组,结果发现,高值组MS发生率显著高于低值组(62.21% vs 54.82%)。正常范围内血清TSH水平与BMI、TG呈正相关,与HDL-C呈负相关,而与FPG、SBP、DBP无相关性,得出结论:甲状腺功能正常人群中,高水平血清TSH对MS组成组分的影响主要是肥胖及血脂代谢异常。不同MS危险因素个数组间相比较,TSH水平无统计学差异(P > 0.05)。
Abstract: Objective: The purpose of this study was to investigate the relationship between serum thyroid stimulating hormone levels (TSH) and metabolic syndrome (MS) with its components in euthyroid young adults. Methods: To use retrospective study method and collected 2576 objects altogether, including 687 euthyroid patients from Ningxia Medical University cardio cerebral vascular disease hospital of physical examination office from September 2016 to September 2017. The normal in-formation of the subjects was collected at the same time, including gender, age, height, weight, body mass index (BMI), fasting plasma glucose (FPG), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), systolic blood pressure, diastolic blood pressure and serum TSH. The corre-lation between serum TSH level and MS and its components in euthyroid young adults was analyzed. Results: In Ningxia area, the incidence rate of MS was 15.37% in euthyroid young adults. The incidence rate of MS was significantly higher in the high normal group of TSH among MS groups (62.21% vs 54.82%). Serum TSH was positively correlated with BMI and TG, and was negatively correlated with HDL-C, of which FPG, SBP and DBP had no correlation. It is concluded that the influence of high normal level serum TSH on MS components is mainly obesity and dyslipidemia. There were no statistical differences in TSH levels between different number risk factor groups of MS (P > 0.05).
