Investigation and Analysis of Off-Label Drug Use in Patients with Chronic Diseases in a Hospital

摘要: 目的：调查我院慢性病患者的超说明书用药情况，分析研究并制定相应的管理对策，为规范超说明书用药，促进合理用药提供依据。方法：筛选HIS系统中2017年12月我院社区门诊慢性病处方2020张，依据药品说明书的规定，从适应症、用药频次、用药剂量和给药途径四个方面进行处方点评，并对点评结果进行统计分析，制定相应的管理对策。结果：慢性病患者中超说明书用药的处方数为688张，超说明书使用率为34.01%。超说明书用药涉及的次数为1030次，其中超适应症531次，占51.55%；超用药频次260次，占25.24%；超用量237次，占23.01%；超给药途径2次。结论:我院社区门诊慢性病患者中超说明书用药情况普遍，存在使用风险，需要制定相应的管理对策，降低用药风险，促进合理用药。

Abstract: Objective: To investigate the situation of Off-label Drug Use in the patients with chronic diseases in our hospital, analyze and study and make corresponding management countermeasure, provide the basis for normalizing the overstatement drug use and promoting the rational drug use. Methods: Screening of 2020 prescriptions for chronic diseases in community outpatient clinics in the HIS system in December 2017. According to the provisions of the drug manual, prescription reviews are made from four aspects: indications, frequency, dosage and route of administration, and the results are statistically analyzed, and corresponding management countermeasures are formulated. Results: The number of prescriptions for out of Off-label Drug Use in chronic diseases was 688, and the rate of Off-label Drug Use was 34.01%. The number of Off-label Drug Use was 1030 times, of which 531 times were over-indication, accounting for 51.55%; Hypermedication frequency 260 times, ac-counting for 25.24%; Excess dosage 237 times, accounting for 23.01%; Hyperdrug route 2 times. Conclusions: For the patients with chronic diseases in the community outpatient clinic of our hospital, Off-label Drug use is common and there is a risk of use. Therefore, corresponding management measures should be formulated to reduce the risk of medication and promote rational drug use.
