全程营养管理配合支持性心理干预对食管癌化疗患者营养指标、负性情绪及不良反应的影响
Effects of Full Nutritional Management Combined with Supportive Psychological Intervention on Nutritional Index, Negative Emotion and Adverse Reactions in Patients Undergoing Esophageal Cancer Chemotherapy
摘要: 目的:探讨全程营养管理配合支持性心理干预对食管癌化疗患者营养指标、负性情绪及不良反应的影响。方法:选择我院2016年9月至2018年8月收治的70例食管癌化疗患者为研究对象,按照随机数字表法分为对照组35例和观察组35例,对照组给予常规护理,观察组在常规护理基础上进行全程营养管理配合支持性心理干预治疗,比较两组患者干预前后体质量、血清清蛋白(ALb)、转铁蛋白(TF)、血红蛋白(Hb)、淋巴细胞数(LYM)、焦虑自评量表(SAS)、抑郁自评量表(SDS)和不良反应变化情况。结果:1) 干预后,观察组体质量、ALb、TF、Hb、LYM水平均高于对照组(P < 0.05);2) 干预后,观察组SAS、SDS评分均显著低于对照组(P < 0.05);3) 干预后,观察组不良反应发生率显著低于对照组(P < 0.05)。结论:全程营养管理配合支持性心理干预治疗可以较好地改善食管癌患者的体质量、ALb、TF、Hb等营养指标,同时缓解其焦虑抑郁等负性情绪,并减少治疗中的相关副反应。
Abstract: Objective: To investigate the effects of full nutritional management combined with supportive psychological intervention on nutritional index, negative emotions and adverse reactions in pa-tients undergoing esophageal cancer chemotherapy. Methods: 70 patients with chemotherapy for esophageal cancer admitted to the hospital from September 2015 to August 2017 were enrolled. They were divided into the control group and the observation group by the random number table method, 35 cases in each group. The control group was given routine nursing. The observation group was given full nutritional management combined with supportive psychological intervention on the basis of routine nursing. The changes of body mass, serum albumin (ALb), transferrin (TF), hemoglobin (Hb), lymphocyte count (LYM), self-rating anxiety scale (SAS), self-rating depression scale (SDS) and adverse reactions in the two groups before and after intervention were compared. Results: 1) After intervention, the body mass, levels of ALb, TF, Hb and LYM of the observation group were higher than those of the control group (P < 0.05); 2) After the intervention, the scores of SAS and SDS in the observation group were significantly lower than those in the control group (P < 0.05); 3) After intervention, the incidence of adverse reactions in the observation group was significantly lower than that in the control group (P < 0.05). Conclusion: The full nutritional management combined with supportive psychological intervention can better improve body mass, ALb, TF, Hb and other nutritional indexes in patients with esophageal cancer, while alleviating negative emotions such as anxiety and depression, and reducing related side effects in treatment.
