Effect of Noninvasive Mechanical Ventilation on Reintubation in Patients with Unplanned Extubation

摘要: 目的：探讨无创机械通气对非计划性拔管患者再插管情况的影响。方法：2015年1月~2017年12月非计划性拔管患者28例，常规应用无创机械通气(NPPV)治疗，计算患者再插管的比例，评价治疗效果。结果：28例患者21例应用无创通气成功并转出ICU，NPPV使用时间(4.7 ± 3.2) d。7例NPPV失败再插管，再插管比例7/28 (25%)，低于文献报道50%~58.3%。结论：无创机械通气有利于降低非计划性拔管患者再插管比例。

Abstract: Objective: To investigate the effect of non-invasive mechanical ventilation on re-intubation in pa-tients with unplanned extubation. Methods: From January 2015 to December 2017, 28 cases with unplanned extubation were routinely treated with non-invasive mechanical ventilation (NPPV). The proportion of patients who were re-intubated was calculated and the treatment effect was evaluated. Results: 21 cases were successfully treated with non-invasive ventilation and transferred out of the ICU. The time of NPPV was (4.7 ± 3.2) d. 7 cases with NPPV were re-intubated, and the rate of re-intubation was 7/28 (25%), which was lower than that reported in the literature by 50% - 58.3%. Conclusion: Non-invasive mechanical ventilation is beneficial to reduce the proportion of re-intubation in patients with unplanned extubation.
