右冠状动脉–冠状静脉窦瘘合并右冠状动脉–右房瘘1例

A Case of Right Coronary Artery-Coronary Venous Sinus Fistula Complicated with Right Coronary Artery-Right Atrium Fistula

摘要: 先天性冠状动脉瘘通常是指冠状动脉主干或其分支与心腔或某一大血管之间存在的异常血管交通，是一种少见的先天性心血管畸形，发病率约占先天性心脏病的0.3%。冠状动脉瘘会影响正常冠脉的血流动力学，因此较易出现诸如心功能不全、感染性心内膜炎、心律失常、心肌缺血等并发症。本文报道了一例临床上较为罕见的右冠状动脉–冠状静脉窦瘘合并右冠状动脉–右房瘘的病例，期望该病例能为临床医生的诊疗提供一定的经验借鉴。患者本人了解并同意本病例报道。

Abstract: Congenital coronary artery fistula usually refers to the abnormal vascular traffic between the main coronary artery or its branches and the heart cavity or a large vessel. It is a rare congenital cardiovascular malformation, accounting for about 0.3% of the cases of congenital heart disease. Coronary artery fistula affects normal coronary blood flow dynamics, and thus some complications more easily appear, such as cardiac insufficiency, infective endocarditis, arrhythmia, myocardial ischemia and so on. This paper reports a case of a clinically rare right coronary artery-coronary venous sinus fistula complicated with right coronary artery-right atrium fistula, hoping the case can offer certain experience for reference for doctor’s clinical diagnosis and treatment. The patient himself understands and agrees with this case report.
