75例乳腺包裹性乳头状癌的临床特征及预后分析
Analysis of Clinical Characteristics and Prognosis of 75 Cases of Encapsulated Papillary Carcinoma
摘要: 目的:探讨乳腺包裹性乳头状癌的临床特征、治疗方式及预后。方法:应用医渡云科研大数据系统(YIDUCLOUD)收集2012年3月至2018年12月青岛大学附属医院收治的75例经手术病理确诊为乳腺包裹性乳头状癌患者的临床资料。结果:75例患者中,女性72例,男性3例,中位发病年龄63岁。原发肿瘤中位大小为2.0 cm。所有患者均行手术治疗。腋窝淋巴结清扫的患者中转移者7例,均为乳腺包裹性乳头状癌伴浸润性癌的患者。中位随访时间43个月(范围:12~93个月),失访6例。5年总生存率为93.7%,5年无病生存率为89.9%。结论:乳腺包裹性乳头状癌是一种极为少见的低度恶性乳腺癌,以手术治疗为主,预后较好。对于伴浸润性癌的包裹性乳头状癌患者建议行前哨淋巴结活检。
Abstract: Objective: To investigate the clinical characteristics, treatment, and prognostic of Encapsulated papillary carcinoma (EPC). Methods: Data of 75 EPC confirmed by operation and pathology patients hospitalized in the Affiliated Hospital of Qingdao University from March 2012 to December 2018 was collected from the YIDUCLOUD system. Results: Of the 75 patients, there were 72 females and 3 males. The median age was 63 years old. The median size of primary tumor was 2.0 cm. All patients received surgical treatment. Among the patients with axillary lymph node dissection, seven of whom had lymph node metastasis. The patients with positive lymph nodes were all cases of EPC associated with invasive carcinoma. The median follow-up time was 43 months (range: 12~93 months), and 6 cases failed to follow up. The 5-year overall survival and disease-free survival rates were 93.7% and 89.9%. Conclusions: EPC is a rare low grade malignant breast cancer, which is mainly treated by surgery and has a good prognosis. We recommend offering sentinel node evaluation to EPC with invasive carcinoma.
