巨大浆膜下子宫肌瘤扭转1例并文献复习

Torsion of Giant Sbserosal Hysteromyoma: A Case Report and Literature Review

摘要: 子宫肌瘤(Uterine fibroids)是女性生殖器官中最常见的良性肿瘤，也是人体中常见的肿瘤之一，但巨大子宫肌瘤扭转较为少见，现报道青岛大学附属医院妇科收治的巨大浆膜下子宫肌瘤扭转1例，子宫肌瘤直径约为20 cm，入院后行急症手术治疗。本文同时对浆膜下子宫肌瘤扭转的相关文献进行复习，认为浆膜下子宫肌瘤蒂扭转属于妇科急腹症的一种，处理原则首选手术治疗，并且对于这种术前辅助检查提示肿瘤直径过大、性质不能确定者，不建议使用旋切器，推荐开腹手术，以确保完整的切除肿瘤，避免恶性肿瘤发生医源性破裂的可能。

Abstract: Uterine fibroids are the most common benign tumor of female genital mutilation, but the reverse is relatively rare. Now reported a case of a huge subserosal hysteromyoma torsion which diameter is about 20 centimeter in Qingdao university affiliated hospital of gynaecology department, emergency surgery was performed after admission. This article aims to review the relevant literature of subserosal hysteromyoma reverse, suppose that subserosal hysteromyoma pedicle torsion belongs to a kind of gynecological acute abdomen, and the first choice of principles of management is surgical treatment, and surgical treatment is recommended for those whose diameter is too large and nature cannot be determined by preoperative auxiliary examination, to ensure that pelvic mass can be removed completely and avoid the possibility of malignant tumor Iatrogenic rupture.
