PHH3在膀胱尿路上皮癌中的表达及临床意义

摘要: 目的:膀胱癌是最常见的泌尿系统肿瘤,发病率及死亡率始终位于泌尿系统肿瘤首位。本研究通过对膀胱正常黏膜组织和尿路上皮癌组织中磷酸化组蛋白H3 (Phosphorylated Histone H3, PHH3)的表达情况,探讨其在膀胱尿路上皮癌中的表达与临床病理特征及预后的关系。方法:回顾性分析2016年1月至2020年1月期间在青岛市市立医院病理诊断为膀胱尿路上皮癌患者87例以及31例正常膀胱黏膜组织,使用免疫组化的方法分别检测PHH3在癌组织和正常粘膜组织中的表达情况,结合临床资料分析其表达与肿瘤的临床分期、病理分级、有无淋巴结转移之间的统计学关系。结果:PHH3蛋白定位于细胞核,呈棕黄色或棕褐色。在尿路上皮癌组织中PHH3表达阳性率为72%,在正常尿路上皮组织中阳性率为7%,差异有统计学意义(P < 0.05)。在尿路上皮癌患者中,PHH3蛋白的表达与临床分期、病理分级、是否淋巴结转移有明显相关性,差异有统计学意义(P < 0.05),与患者年龄、性别未见明显关系,无统计学意义(P > 0.05)。PHH3蛋白在一定程度上能判断膀胱癌进展状态和恶性程度,有较高的预后评估价值。
Abstract: Objective: Bladder cancer is the most common urinary system tumor, and its morbidity and mortality always rank first in urinary system tumors. In this study, through the expression of Phosphorylated Histone H3 (PHH3) in normal bladder mucosa tissues and urothelial carcinoma tissues, the relationship between the expression of Phosphorylated Histone H3 (PHH3) and clinicopathological characteristics and prognosis was investigated. Methods: A retrospective analysis of 87 patients with bladder urothelial carcinoma and 31 normal bladder mucosal tissues were pathologically diagnosed in Qingdao Municipal Hospital from January 2016 to January 2020. Immunohistochemistry was used to detect the expression of phh3 in cancer tissues and normal mucosa tissues. Combined with clinical data, the statistical relationship between its expression and clinical stage, pathological grade and lymph node metastasis was analyzed. Results: PHH3 is expressed in the cell nucleus and is brown or tan. The positive rate of PHH3 expression was 72% in urothelial carcinoma tissues and 7% in normal urothelial tissues, and the difference was statistically significant (P < 0.05). In urothelial carcinoma patients, the expression of PHH3 protein was significantly correlated with the clinical stage, pathological grade and lymph node metastasis, with statistical significance (P < 0.05). There was no significant relationship between age and gender of patients and no statistical significance (P > 0.05). PHH3 protein can predict bladder cancer progression and malignancy to some extent, and has high prognostic value.
