Abstract: Objective: To investigate whether primary tumor surgery can improve the prognosis of patients with distant metastatic soft tissue sarcoma of the extremities. Methods: Data of patients with distant metastases of soft tissue sarcoma of extremities from 2010 to 2015 were selected from SEER database. Propensity score matching scores were used to balance differences between surgical and non-surgical patients. Kaplan-Meier method was used to study the difference in prognosis of each group of patients, and log-rank test was used for statistical analysis. Results: A total of 8331 patients were enrolled in this study. Survival analysis showed that primary site surgery significantly improved the prognosis of patients with distant metastatic soft tissue sarcoma of the extremities, while limb salvage and amputation had no effect on the prognosis of patients with distant metastatic soft tissue sarcoma of the extremities. Conclusion: Primary site surgery significantly improved the prognosis of patients with distant metastatic soft tissue sarcomas of the extremities.
