儿童坏死性肺炎预测指标的研究进展

摘要: 坏死性肺炎(NP)是肺炎的一种较严重并发症，胸部CT检查是其诊断的金标准，但胸部CT检查有辐射损伤，价格较高，短期内不宜反复进行。热程、WBC、NE%、hs-CRP、PCT、LDH、红细胞沉降率、D-二聚体、IFN-γ是坏死性肺炎发生的独立预测指标。肺炎支原体坏死性肺炎患儿肺部病变的平均CT值<7.25 pg/ml时，需警惕为坏死性肺炎。支气管镜下痰栓堵塞需要活检钳清除以及肺泡灌洗液呈浑浊的米汤样是坏死性肺炎的独立预测因素。支气管镜下黏膜糜烂是肺炎支原体坏死性肺炎的独立预测因素。
Abstract: Necrotizing pneumonia (NP) is a serious complication of pneumonia, and chest CT examination is the gold standard for its diagnosis. But the CT examination has radiation damage, high price, and it should not be repeated in the short term. Length of fever, WBC, NE%, hs-CRP, PCT, LDH, erythrocyte sedimentation rate, D-dimer and IFN-γ are independent predictors of necrotizing pneumonia. When the mean CT value of pulmonary lesions in children with Mycoplasma pneumoniae necrotizing pneumonia is less than 7.25 pg/ml, it is necessary to be alert to necrotizing pneumonia. The presence of sputum bolt under a bronchoscope which needs to be removed with biopsy forceps and rice-water-like bronchoalveolar lavage fluid may be the early predictors of necrotizing pneumonia in children. Bronchoscopy mucosal erosion is an independent predictor of necrotizing pneumonia in Mycoplasma pneumoniae.
