摘要: 目的:通过Meta分析,评价直流电刺激对卒中后患者认知功能的影响分析。方法:通过检索中国知网(CNKI)、维普(VIP)、中国生物医学文献库(CBM)、万方(WanFang)、EMbase、Web of Science、PubMed和The Cochrane Library数据库。检索数据库中有关于直流电刺激对卒中后患者认知功能影响的随机对照试验的所有相关的文献,检索时间从建库至2022年2月25日。根据设定的纳排标准进行筛选、提取所需数据并对文献进行质量评价,最终采用RevMan5.4进行Meta分析。结果:研究最终共纳入10篇文献,观察组347例,对照组344例,共691例。Meta分析结果显示:直流电刺激治疗方式可以改善卒中后患者的认知功能,Meta分析结果有统计学意义:Meta分析结果也表明实验组患者的有效率、Mini-Mental State Examination (MMSE)、蒙特利尔认知评估量表(MOCA)、Fugl-Meyer(FMA)评分与对照组比较有统计学意义,有效率[WMD = 1.23, 95%CI (1.12, 1.34), P < 0.00001]; MMSE: [MD = 2.82, 95%CI (2.25, 3.38), P < 0.00001]; MOCA: [MD = 2.10, 95%CI (1.75, 2.44), P < 0.00001]; FMA: [MD = 6.64, 95%CI (2.74, 10.54), P = 0.0008]。结论:直流电刺激治疗方式可以有效改善卒中后患者的认知功能,该结论将需要更多样本来进行RCT研究来进一步确认其疗效。
Abstract: Objective: To evaluate the effect of direct current stimulation on cognitive function in patients after stroke by meta-analysis. Methods: CNKI, VIP, China Biomedical Literature Database (CBM), Wan-Fang, EMbase, Web of Science, PubMed and The Cochrane Library databases were retrieved. All relevant literature in the database of randomised controlled trials on the effects of DC stimulation on cognitive function in post-stroke patients was searched from the establishment of the library un-til 25 February 2022. According to the set criteria for screening, extracting the required data and evaluating the quality of the literature, revMan 5.4 was finally used for meta-analysis. Results: A total of 10 articles were included in the study, including 347 cases in the observation group and 344 cases in the control group, for a total of 691 cases. Meta-analysis results showed that DC stimulation treatment modalities can improve cognitive function in post-stroke patients, and meta-analysis re-sults are statistically significant. Meta-analysis results also showed that the experimental group pa-tients were effective. Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment Scale (MOCA), Fugl-Meyer (FMA) scores compared with the control group were statistically significant. Ef-ficiency [WMD = 1.23, 95%CI (1.12, 1.34), P < 0.00001]; MMSE: [MD = 2.82, 95%CI (2.25, 3.38), P < 0.00001]; MOCA: [MD = 2.10, 95%CI (1.75, 2.44), P < 0.00001]; FMA: [MD = 6.64, 95%CI (2.74, 10.54), P = 0.0008]. Conclusions: Direct current stimulation treatment can effectively improve the cognitive function of patients after stroke, which will require more sample size and high-quality lit-erature for randomized controlled trials.
