摘要: 颞下颌关节(Temporomandibular joint, TMJ)脱位是指下颌骨髁突超出正常活动的最大限度。延迟治疗时间越长,复位越困难,颞下关节脱位复发风险越高。在此,我们强调最好的选择是不延迟诊断和早期颞下颌关节复位。颞下颌关节脱位的治疗以手法复位为主。对于已经存在很长时间的脱位,手工复位可能是无效的,往往需要手术治疗。对于复发性脱位可采用微创技术(肉毒毒素注射、自体血注射等)治疗,若这些方法无效,则需手术治疗。颞下颌关节脱位的最佳治疗方案应综合考虑脱位的原因、患者的年龄、依从性、治疗目标等因素。在这里,我们需要再次强调早期重置的重要性。
Abstract: Temporomandibular joint (Temporomandibular Joint, TMJ) dislocation means that the mandibular condyle exceeds the maximum limit of normal motion. The longer the treatment is delayed, the more difficult it is to reposition, and the higher the risk of recurrence of infratemporal joint disloca-tion. Here, we emphasize that the best option is not to delay diagnosis and early mandibular reduc-tion. The treatment of temporomandibular joint dislocation is mainly manual reduction. For dislo-cations that have existed for a long time, manual reduction may be ineffective and surgery may be required. Recurrent dislocation can be treated with minimally invasive techniques (botulinum tox-in injection, autologous blood injection); if these methods are ineffective, surgical treatment is re-quired. The best treatment plan for temporomandibular joint dislocation should comprehensively consider the cause of the dislocation, the patient’s age, compliance, treatment goals, and so on. Here, we need to emphasize the importance of early reset.
